Who are we?
The Transitions of Care and Primary Care Specialty Practice Group is a network of SHPA members who have an interest in strengthening the robustness of the Australian healthcare system in providing continuity of care, including medication management, at discharge from hospital into primary care. Our members may work in hospital-based services or programs including Transition Care, HARP, Hospital in the Home, HOMR or be based in primary care working in General Practice, Community Pharmacy, Home Care Services, Aged Care and/or as independent accredited pharmacists.
